SCEA Event Registration Form
This is a generic form for all SCEA events. It can be used for as few as one and as many as ten from a single district association. Please be sure to fill out all the requested information.
The person filling out the information must fill out the information requested. All fields are required.
First list the contact information of the one filling out the form
Only one event can be registered per submission of this form.
Second section is how many total are attending based on this registration form
Third is a list of those who are registering to attend. This includes the one filling out the form if the yare attending. Please leave the unused list of attendee spaces blank.
Fourth, Enter the name of the event.
Fifth enter the date of the event.
The multiple district registration is for convenience. If members want to register separately they can except for the LAT Reception. In the case of LAT Reception, the President should register the guests.

Cancelations after registration has been submitted must be made within three (3) days of the event or the district may be charged. Please mark your calendars.
